Job Title : Registered Nurse (RN) Job Specialty : Extended Care Job Duration : 14 Weeks Shift : Day Shift - 5x8 Hr Guaranteed Hours : 36 Hours per Week Experience : Not Specified License : Current license as a Registered Nurse in the state of practice Certifications : BLS - American Heart Association Must-Have : - ASN - Graduation from an accredited School of Nursing - Able to read and write to fulfill documentation requirements - Physical ability to lift objects over 100 lbs with frequent lifting / carrying up to 50 lbs Job Description : - Utilize the nursing process, including assessment, diagnosis, planning, implementation, and evaluation - Advocate for residents to ensure they have adequate information to make health decisions - Coordinate total care and work with residents to achieve or maintain their highest practicable level of function - Ensure the quality of resident care meets legal and professional standards - Complete holistic, age-specific admission history, physical assessment, and nursing plan of care - Analyze and incorporate clinical data to formulate an individualized plan of care - Identify and report signs / symptoms of neglect / abuse related to child, domestic, or elder abuse - Set priorities for and review daily plan of care in collaboration with resident / family / significant others - Communicate with physicians to ensure coordination of medical and nursing plans of care - Demonstrate competence in performing technical skills, including medication and IV administration - Document nursing interventions, residents' response to care, and resident outcomes - Evaluate data to revise the plan of care to meet the changing needs of the resident - Identify and document resident / family learning needs and provide education - Identify and document discharge planning needs and coordinate or delegate discharge activities - Maintain knowledge of CMS regulations, Medicare skilled, and MDS criteria for documentation compliance - Supervise, coach, counsel, and evaluate the performance of assigned C.N.A. staff
