Advanced Practice Provider - Home Care
We are seeking experienced Nurse Practitioners and Physician Assistants to join our growing team providing in-home care services in the Orlando, FL community.
Our providers work with DHMTs to deliver comprehensive health assessments, diagnosis, treatment, and outcome management for patients of all ages.
This role involves traveling to the patient's location and providing point-of-care laboratory studies, minor procedures, wound care, and more.

Responsibilities :
- Perform comprehensive health assessments and diagnose / treat complex illnesses
- Provide therapeutic interventions and minor procedures
- Use critical thinking skills and follow evidence-based standards of practice
- Accurately and thoroughly document patient encounters
- Analyze test data to diagnose and arrange appropriate follow-up
- Educate patients and families on acute illness treatment and management
- Effectively communicate with patients, families, and other healthcare professionals
- Adhere to clinical and safety standards and performance metrics
- Provide care with compassion, empathy, and cultural competency
- Lead practice with a focus on patient care
Requirements :
- Minimum of two years of experience in relevant healthcare settings
- Graduate with an advanced degree from an accredited institution
- Current unrestricted state license as a Nurse Practitioner or Physician Assistant
- Nationally board certified and have prescriptive authority and DEA
- Valid driver's license with clean driving record
- Ability to lift and carry equipment up to 50 pounds
- Ability to work a varied schedule with evenings, weekends, and holidays
