Seeking Palliative Care Nurse Practitioner to join an expanding Inpatient Palliative Care (Hospice) team.
inpatient rounding on hospice patients.
- Monday Friday.
- Support 6 patients per day.
- 5 day workweek, 8am 5pm, rotating on-call. Practice autonomously.
- Assess, plan, implement, and evaluate care needs.
- Symptom management, chronic disease management, goals of care, advanced care planning.
- EPIC EMR
- Join a non-profit tertiary referral center with physicians.
- 2 hospitals with -bed facility.
- Level II Trauma center.
- DaVinci robotics.
- Academic affiliation of Osteopathic Medicine.
- Inpatient experience in Palliative Care or Hospitalist role required
