Primary Care Physician – Jacksonville, FL

Are you a physician passionate about making a real difference in the lives of seniors? Client is seeking a dedicated Primary Care Physician (PCP) to join our mission-driven, physician-led team. Our focus is on transforming care for the neediest population by delivering affordable, VIP-level care that truly improves health outcomes.

As a PCP at Client , you’ll :

  • Manage your own panel of patients , taking full accountability for their care, outcomes, and overall health.
  • Act as a health coach , helping patients set and achieve meaningful short- and long-term health goals.
  • Lead a collaborative care team , including care promoters, facilitators, and coordinators, ensuring seamless, high-quality care.
  • Engage across care settings , from in-office visits to hospitals, skilled nursing facilities, and homebound patients.
  • Champion a positive culture , fostering teamwork, accountability, and a passion for patient-centered care.

What we’re looking for :

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar.
  • Board eligible (certification preferred) and active / eligible state medical license.
  • DEA license for prescribing controlled substances and BLS certification.
  • Experience providing primary care for older adults; exposure to value-based care or managed care is a plus.
  • Strong leadership, coaching, and communication skills, with a commitment to outcomes-focused care.
