LPN’s provide a large portion of direct patient care. LPN’s may be assisted by nurses’ aides (CNAs) and other assistants in some of their duties.

LPNs are directed by doctors and nurses (RNs & nurse managers).

Qualifications :

• tGraduate of an accredited school of nursing, or proof of successful challenge of LPN Exam.
• tMinimum of one (1) year recent experience in a home care setting within the period of two (2) years.
• tCurrent unrestricted LPN license in the State of Maryland
• tCurrent CPR

Job Duties :

• 1. tAssisting in assessing, planning, implementing and evaluating patient care;
• 2. tConducting and documenting ongoing data collection related to patient / family reporting to the RN following guidelines, policies and procedures;
• 3. tProviding care to age specific patient population such as adolescent, adult or older adult-utilizing the nursing process;
• 4. tProviding direct or indirect patient care services including but not limited to medication administration;
• 5. tDemonstrating the ability to perform and respond appropriately in emergency situations;
• 6. tCompleting nursing care documentation for assigned patients;
• 7. tDeveloping and maintaining effective communication with patient, families, physicians, colleagues and co-workers, to ensure the on-going provision of patient care;

8. tPerforming tasks and / or interacting with patients, families, staff and other personnel in a courteous, honest and compassionate manner.

9. tObserve patients, charting and reporting changes in patients’ conditions, such as adverse reactions to medication or treatment, and taking an necessary action.

10. tAdminister prescribed medications

11. tAnswer patients calls and determine how to assist them

12. tMeasure and record patients’ vital signs, such as height, weight, temperature, blood pressure, pulse and respiration.

13. tHelp patients with bathing, dressing, and personal hygiene, moving bin bed, and standing and walking.

14. tWork as a part of a health care team to assess patient needs, plan and modify care and implement interventions.

15. tRecord food and fluid intake and output

16. tEvaluate nursing intervention outcomes, conferring with other health-care team members as necessary.
