About The Position
Job Title : Rehabilitation Therapy Assistant Job Specialty : Certified Occupational Therapy Assistant (COTA) Job Duration : 13 Weeks Shift : Monday - Friday Guaranteed Hours : 32 per week Experience : Open to New Graduates;
SNF Experience Preferred License : State License Required Certifications : COTA Certification Must-Have : Prescreen Form, Resume, License Verification, ReferencesJob Description : - Ensure all prescreen forms are completed and attached for submission.
- Attach a resume and license verification for application consideration.- Conduct a COVID test 72 hours prior and obtain results 48 hours before the start date.
- Complete credentialing checklist including : - State License Verification - Professional References (3) - County and Federal Criminal Background Check (7 years) - Multi-State Criminal Search (7 years) - Nationwide Sex Offender Search (7 years) - HHS / OIG Search - GSA Search - Social Security Number Trace - Complete a 2-step PPD or Quantiferon, or provide proof of positive PPD and chest X-ray - SAM Search - Oregon State Background Check (facility run)- No holiday coverage required.
