We are seeking a LPN for a full-time and 100% Home Health focused opportunity in the Golden / Lakewood area, CO!

Compensation Package :
- $30 to $35 Per Regular Visit

Requirements :
- Clear and Active LPN License in Colorado
- 1+ Years of Experience as an LPN
- Current and Active Driver’s License
- Home Health Experience Preferred - Training Offered
Responsibilities :
- Monday through Friday 8 AM to 5 PM
- 1 Saturday and Sunday Every 4 Weeks for Additional Compensation OR 2 Days Off
- Working with Geriatric Patients
- Using Matrix Care EMR - Training Offered
- Seeing 6 Geriatric Patients Per Day in Their Home - Routes Are Coordinated Depending on Your Home Location
- 1 to 2 Weeks of Onboarding / Orientation
- Applies nursing knowledge in planning, implementation, and evaluation of human response to actual or potential health problems
- Communicates case findings and initiates health teachings, health counseling, and provision of care that supports life or restores well-being
- Functions under the direction of the Nursing Supervisor
