About The Position
Job Title : Physical Therapist Job Specialty : Rehabilitation Therapy Job Duration : 13 weeks Shift : Monday to Friday Guaranteed Hours : 32 hours per week Experience : New graduates are welcome;
SNF (Skilled Nursing Facility) experience preferred License : State license verification required Certifications : None specified Must-Have : - Prescreen completed and submitted- Resume- License verification- Covid test 72 hours prior to start date with results issued 48 hours prior to start date Job Description : Complete and attach prescreen, resume, and license verification upon submission.
- Provide three references.- Undergo county criminal background check for the past 7 years for all locations lived and worked.
- Federal criminal background check for 7 years or more.- Multi-state criminal search for 7 years or more.- Nationwide sex offender search for 7 years or more.
- HHS / OIG search and GSA search.- Social Security Number trace.- Complete a 2-step PPD or Quantiferon test, or provide proof of positive PPD along with a chest X-ray.
- SAM search.- Undergo Oregon State background check - CRIMS (facility run).Please make sure all requirements are met to avoid candidate disqualification.
This position does not require holiday coverage, and it is not urgent. Bilingual capability is not required.
