Recovery Support Specialist - Assertive Community Treatment

Position Summary :

Under the general supervision of the Supervisor, ACT will provide behavioral health assessment and treatment planning, case management, support, education, and consultation for individuals enrolled in the program.

Will provide mobile crisis response and intervention to ACT clients. Function as an effective team member and represent the agency in a professional manner.

Provides peer services to ACT clients and serves as the Peer for the ACT Team.

Qualifications :

CERTIFICATION & LICENSURE REQUIREMENTS

Certified Recovery Support Specialist (CRSS) within 18 months or Certified Family Partnership Professional (CFPP) (IAODAPCA) within 18 months and Driver's License upon hire.

EXPERIENCE REQUIREMENT

Lived experience in recovery.

Demonstrates all of the following :

  • Skill in the delivery of rehabilitative services to adults or children;
  • The ability to work within a provider agency's structure and accept supervision.
  • The ability to work constructively with individuals receiving services, other providers of service, and the community.

SKILLS AND KNOWLEDGE

High level or organization and adaptability, Occasional exposure to unpleasant client or unit elements, Contact with clients under wide variety of circumstances.

May be exposed to / occasionally exposed to client elements. Exposed to unpleasant elements (accidents, injuries and illness).

Occasionally subjected to irregular hours. Requires judgement / action which could result in serious consequences. Occasional pressure due to multiple calls and inquiries.

Pre-planning with daily scheduling.

Essential Functions :

Provides community-based / in-home behavioral health services for individuals experiencing serious mental illness that interferes with their ability to manage mental health symptoms and live independently.

  • Provides transportation to client as clinically appropriate.
  • Completes or contributes to assessment and treatment plan.
  • Request consultation, where indicated, in a diligent and timely manner.
  • Provides on-call interventions for ACT clients, covering after business hours, overnights and weekends as assigned.
  • Provides crisis assessment and intervention, completes safety plan with client, and follows up with client following crisis situation.
  • Coordinates with Mobile Crisis Response and other crisis service teams when clinically appropriate.
  • Provides individual and group ACT services to include, but not limited to, monitoring of daily living activities, assistance maintaining benefits and entitlements, assistance with employment and education, support to manage symptoms, medication observation as requested by physician, advocacy for client's rights and preferences, assistance accessing resources, family support, coordination of services and care.
  • Maintains records in accordance with policy, as well as state and federal oversight agencies and accreditation agencies including Joint Commission, Medicaid, Medicaid Rehabilitation Option, Illinois Department of Public Health, and Commercial Insurances.
  • Acts as a liaison and resource between the community and client needs; coordinates treatment goals with community agencies, primary care physicians, court system, and managed-care entities.
  • Act as a peer within the team.
  • Discloses lived experience to clients as appropriate in the treatment process.
  • Utilizes personal experiences in recovery to assist clients in their own recovery and treatment progress.
  • Demonstrates organization Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and / or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, Joint Commission, OSHA and other federal / state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse.
  • Brings any questions or concerns regarding compliance to the immediate attention of administrative staff.
  • Takes appropriate action on concerns reported by department staff related to compliance.
  • Participates in in-service and competency training.
  • Participates in interdisciplinary team meetings.
  • Prepares special reports as needed on activities, and work group activities.
  • Meets clinical direct service standards.
  • Meets compliance standards for all documentation and clinical charts.
