About The Position
Job Title : Care Coordinator Social Worker Job Specialty : Care Management Job Duration : 13 weeks Shift : Day, 4x10 Friday, Saturday, Sunday, Monday (Weekend and holiday rotation) Guaranteed Hours : Not specified Experience : Minimum 1 year in healthcare in an acute care setting or related field, or community or educational internship in care coordination.
A background in medical social work in an acute care setting is preferred. License : Social Work License Certifications : Basic Life Support (BLS) Must-Have : - Bachelor's Degree in Social Work or a Master's Degree in Social Work from an accredited college or university - Excellent written and verbal communication skills Job Description : - Responsible for providing psychosocial assessments for patients, including timely and appropriate planning to advance the discharge plan.
- Assist with relaying information about community-based service offerings and guidance for patients / families dealing with multi-system factors impacting psychosocial dynamics.
- Serve as a specialist on issues related to psychosocial and discharge needs, including end-of-life care planning. - Partner and offer feedback to the RN Case Manager concerning complex social determinants of health issues, situational dynamics, and social needs.
- Implement discharge planning and provide resource information timely and efficiently. - Identify and document barriers to timely disposition.
- Understand eligibility processes and criteria for both private and public resources for safe transitions. - Respond to referrals for patient assistance from care team members.
- Participate in interdisciplinary rounds to ensure care transition plans are appropriate. - Initiate and facilitate post-acute referrals for timely transitions.
- Conduct financial needs assessments for patients requiring follow-up care. - Advocate for and partner with patients and families in autonomous health care decision-making.
- Account for cultural or religious beliefs in service provision and continuity of care. - Participate in developing protocols, procedures, and performance improvements for optimized outcomes.
- Ensure timely and accurate documentation of progress and interactions. - Track avoidable days and report trends leading to undesired outcomes.
- Complete all professional competency assessments, mandatory education, and required population-specific education. - Serve as a preceptor or mentor for social work students, if appropriate.
