General Summary of Position
The Hospice Liaison, a Registered Nurse (RN), will act as liaison between the hospital and the hospice team, make inpatient visits to hospice patients, act as consultant for pain and symptom management, provide in-service education to providers, perform pre-admission evaluations for appropriateness for hospice services, initiates the admission process by educating and informing patient / family of hospice and it's services.
The liaison will ensure the provision of quality patient care and customer services. Assists Director in developing policy and procedure to improve patient outcomes.
Primary Duties and Responsibilities
- Provides admission assessment and engages with patients and families to develop rapport and a sense of trust. Establishes the hospice plan of care that includes appropriate Physician orders for pain and symptom management.
- Provides a clear explanation that this is limited stay for symptom management. Communicates with Social Work (SW) to initiate a plan for discharge once the symptoms are managed.
- If a patient is to be admitted or transferred to home or a facility, coordinates admission with the Hospice Intake / Referral department.
- Conducts death visits and assists in the assessment of family needs for bereavement services following the patient's death and conveys information to bereavement staff.
Completes documentation of death in the EMR.
- Completes all electronic documentation in a timely manner in both the hospice EMR and hospital EMR
- Case Manages patients who are GIP level of care which includes assuring the patient receives a daily Skilled Nursing Visit (SNV).
Documentation of daily SNV will include the symptom being managed, the intervention and the discharge plan.
Participates in the hospice on call schedule two shifts a month, as needed.
Minimum Qualifications
Education
Bachelor's degree from an accredited school of nursing required
Experience
Experience in hospice care required
Licenses and Certifications
- RN - Registered Nurse - State Licensure and / or Compact State Licensure in Maryland or multistate compact license required
- CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required
- Additional unit / specialty certifications may vary by department or business unit.
This position has a hiring range of $32 - $47.79
