Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. Founded in 1979, we are a local, independent nonprofit that has provided thousands of patients and their families with physical, emotional, and spiritual support throughout the journey from a life-threatening diagnosis through death and bereavement.

Summary

We are seeking a compassionate and skilled Nurse Practitioner to join our team in a hybrid Hospice and Palliative Care role. This position provides both consultative palliative care and hospice medical services, ensuring high-quality, patient- and family-centered care across home, facility, and inpatient settings. This role serves as an NP in accordance with BTBH organizational policies and procedures and applicable laws and regulations.

Schedule

Monday - Friday.; 8 : 30am-5 : 00pm; Hybrid

Location : San Mateo County

  • Bi-lingual in Spanish, Cantonese, Mandarin preferred

Mission Hospice & Home Care follows all CDPH vaccine requirements for healthcare personnel.

Work Environment

  • Hybrid role with visits in home, facility, and inpatient settings
  • Travel within service region required; valid driver’s license and reliable transportation needed
  • Some evenings, weekends, and on-call coverage required
  • Key Responsibilities

  • Deliver bedside assessments, pain and symptom management, and goals-of-care conversations for patients with serious or life-limiting illness
  • Conduct hospice face-to-face visits and document eligibility in compliance with CMS regulations
  • Collaborate with the interdisciplinary team to develop and update care plans
  • Educate patients, families, caregivers, and community providers on hospice and palliative care
  • Ensure accurate and timely documentation to support quality care and regulatory requirements
  • Education / Experience

  • Bachelor’s degree and graduate of accredited Nurse Practitioner Program or Master’s in clinical field of nursing or clinical field related to nursing
  • Experience in an acute care, home health, palliative or hospice setting preferred
  • Certificates, Licenses, Registrations

  • Advanced practice Registered Nurse with active Nurse Practitioner license
  • Public Health Nursing certification is highly desirable
  • HPNA certification preferred
  • Pay Range

    $70.00 - $80.00 hourly
