JOB RESPONSIBILITIES :
Assessment :
a. Initiate a nursing assessment of patient’s condition upon admission.
b. Recognize and assess initial alterations to the patient’s body systems and communicate with clinical team.
c. Organize and record assessment data for accuracy, completeness, and confidentiality.
d. Assess, stage, and measure wounds using photographic technology accurately and in accordance with policy and procedure.
e. Identify patient care situations that require intervention, implement nursing actions, and evaluate responses.
f. Identify educational and discharge needs to patients and their families and implement a teaching plan of care.
Planning :
a. Collaborate with Staff RN Case Manager to identify the patient's present and potential problems from the assessment.
b. Implement a teaching plan in collaboration with Staff RN / Case Manager.
c. Prioritize problems according to impact on health status.
d. Formulate with Staff RN / Case Manager the desired outcomes that are specific and measurable within a certain time frame and consistent with other health provider's expectations.
e. Assess home health care needs, complete a support system evaluation, and assess patient teaching needs from admission to discharge.
Communicate information to Staff RN / Case Manager.
Implementation :
a. Participate in implementing planned changes and activities to improve care.
b. Hold self-accountable for the delivery of quality nursing care.
c. Promote harmonious relationships and favorable attitudes among team members.
d. Implement wound care and HBO protocols when clinically indicated.
e. Individualize care according to age specific population.
Evaluation :
a. Evaluate achievement or lack of achievement of desired outcomes.
b. Collaborate with providers and the clinical team to ensure plans of care are appropriate, revising as necessary, and continually evaluating until desired outcomes are realized.
c. Complete 4-week reviews in a timely manner and ensure all recommendations to the plan are activated in the provider orders.
General :
a. Function as an intake / discharge nurse during clinic times.
b. Foster continuity of care between the Wound Care Department and caretaker (home, hospital, home health, etc.).
c. Prepare clinic records to enable accurate and complete updates to the physician.
d. Assist others within the clinic.
e. Maintain open, professional communication with patients, physicians, and staff.
f. Safely operate all clinical equipment.
g. Utilize appropriate infection control procedures.
h. Maintain skills and knowledge through training and continuing education. Complete mandatory education requirements. Attend staff meetings and in-services.
i. Monitor the condition of equipment and report malfunctions to the Supervisor or Director.
Performs additional related duties as assigned. Clearly communicates and continuously supports the Mission and Values of JMH.
Conducts all activities in compliance with applicable laws, regulations, standards and JMH policies and procedures including Blood and Body Substance Precautions.
EDUCATION, EXPERIENCE AND TRAINING :
Must be licensed in the state of Indiana as a Registered Nurse. At least 2 years of Medical / Surgical experience required.
Wound care experience preferred. Current BLS or CPR for Healthcare Providers certification required.

Monday, Tuesday, Friday - 8 : 00 a.m. - 5 : 00 p.m.
48 hours per bi-weekly pay.
