Competitive Pay :

• $20.00-$21.00 / hr + credit for experience Flexible Schedule : This position is full time; typically scheduled Monday through Friday from 8 : 00 AM to 4 : 00 PM, with occasional evening and weekend availability required Supportive Team :   We value our team as much as our clients Quick Hiring :   Apply today and hear back within 48 hours What You'll Do : Advance the Ministry, Mission, and Core Values of The Lutheran Home and Lutheran Life Communities through professional, ethical practices and exceptional hospitality Manage intake calls and follow up on all incoming leads Actively promote MySolutions Home Care services throughout Lutheran Life Communities, with a focus on the LH campus Visit new arrivals in MyRehab shortly after admission and again prior to discharge to ensure smooth care transitions Assess client needs and develop individualized service plans Oversee the delivery of MySolutions home care and handyman services Conduct regular home visits, 90-day check-ins, annual care plan reviews, and ongoing assessments Build and maintain strong relationships with clients, families, and caregivers Take a holistic approach by integrating direct and indirect services Connect clients to community resources and advocate on their behalf Assist with caregiver scheduling and onboarding Train new staff and coordinate quarterly and annual training sessions What You'll Need : College degree preferred Sales, Healthcare or Social Work background a plus Supervisory experience a plus Must be able to read, comprehend and write legibly in English Must be able to comprehend and perform oral and written instructions and procedures Must be able to calculate math figures and amounts To apply, please complete the required questionnaire.
