One of our top clients is seeking a dedicated LPN to join our growing home health team! In this role, you will play a vital part in ensuring our patients receive the highest quality care in the comfort of their own homes.
About the Schedule :
- M-Th schedule :
- Option 1 : 10 : 00 pm - 6 : 00 am
- Option 2 : 11 : 00 pm - 7 : 00 am
About the Role :
- Provide skilled nursing care to patients in their homes, following established plans of care.
- Perform assessments, monitor vital signs, administer medications, and manage wounds.
- Educate patients and families on self-care and disease management.
- Collaborate with the care team, including physicians, therapists, and social workers.
- Document care accurately and efficiently.
- Maintain a professional and compassionate demeanor.

Qualifications :
- Graduate of an accredited LPN program.
- Current Washington State LPN license.
- BLS certification (must be obtained within 30 days of hire).
- Minimum of one year of experience in home health or a related setting (preferred).
- Strong assessment, communication, and interpersonal skills.
- Ability to work independently and as part of a team.
- Reliable transportation and a valid driver's license.
