Compensation : At Guaranteed, we aim to offer equitable and competitive compensation that demonstrates the value we place in our employees and our appreciation for the work they preform;

the base rate for this role will likely fall between $35-$45 per hour. This role is likely to grow into a full-time position that will include a full range of benefits.

Summary

Guaranteed is seeking to add passionate and empathic social workers to our growing Hospice Care Teams across the Los Angeles region.

Our Social Workers provide emotional and practical support to patients and caregivers for patients who are nearing the end of their lives.

You will support patients and their relatives in their homes, under the leadership of a clinician, in adapting and planning multidisciplinary ways of treating the caregiver system.

Our social workers are put up as an integrative service to make a patient’s sickness and suffering at the end of its life more bearable.

During the patient’s challenging times, you will offer mental, physiological, and moral support. This job requires the capacity to perform in a continuous state of vigilance while remaining safe.

Job Responsibilities

Provide patients and their dear ones with empathetic support

Educate families and assist them in locating local programs and resources for practical and environmental needs

Make visits to the patient’s house (or virtually) and spend some quality time with them and their family

Use advocacy, compassion, and problem-solving abilities to create calmness to stressful situations

Maintain clinical records on all patients

Serve as liaison between patients and families / caregivers and community agencies

Maintain collaborative relationships with organization personnel to support patient care

Actively participate in quality assessment performance improvement teams and activities

Assist family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities

Skills and Toolset You’ll Need to Bring

Deep knowledge in assessing psychosocial status of patients and families / caregivers related to the patient's terminal illness and environment

Finesse in conducting social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse

Experience developing intervention plans based on evaluation findings

Background in crisis intervention

Comfortability in collaborating and communicating with registered nurse and other members of the interdisciplinary group

Possesses and maintains current CPR Certification.

Must be licensed with an automobile that is insured in accordance with state and / or organization requirements and is in good working order.

Nice to Have

Experience in coordinating various activities for patients and their families, which may include arranging transportation to and from appointments, helping patients apply for government benefits or insurance programs, and assisting family members with paperwork related to funeral arrangements

Culture sensitivity : you can assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family.

Including communication, role of family, space, and any special traditions or taboos

Relevant Experience

Master's degree in social work, psychology, or sociology

Licensed Social Worker in the state of California

Prior hospice experience

At least one year of experience in health care

Understands hospice philosophy, and issues of death / dying

Demonstrates good verbal and written communication, and organization skills
