Our medical practice is looking for an outstanding full-time Facility Liaison to join our field team providing assistance to our Physicians and Mid Level Providers in skilled nursing and assisted living facilities.
This role requires : Scheduling visits, completion of charts with H&P, labs, vitals, assisting the facility and provider with needs in the facility.
This position is for the Winter Haven area and includes traveling facility to facility.

- Mobile travel to facilities
- Interview and verify patient information and record medical history, confirming purpose of visit.
- Prepare patients for examination by performing vitals and reporting patient history summary.
- Secure patient information and maintain patient confidentiality by HIPPA Compliance.
- Counsel patient or Facility Staff by transmitting physician's orders.
- Coordinating the daily administrative activities, including scheduling, prepare EMR charts, medicine reconciliation, and consent forms.
- Maintain safe, secure, and healthy work environment by establishing and following standards and procedures, complying with legal regulations.
- Enhance practice reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.
