Location : Resources for Human Development Program Sites in Bryn Mawr and Norristown / Conshohocken

Salary : $22.20 per Hour

Employment Type : Full Time : Monday-Friday 9am-5pm

$2000 SIGN ON BONUS

About the Positions

The Behavioral Health Clinical Coordinator is responsible for organizing patient care activities and sharing information among all the participants concerned with a patient’s care to achieve safer and more effective treatment outcomes. In addition, the Behavioral Health Clinical Coordinator oversees the supervision of support staff, including Care Coordinators and Resource Coordinators.

There are two Clinical Behavioral Health Coordinator positions available. One position will be working at a site in Bryn Mawr, PA. The other position will split their time between a site in Norristown and another site in Conshohocken, PA.

About the Job Sites

New Foundations, within Montgomery County Residentials, is comprised of group homes for adults with mental illness. The program is located in the community and provides supervision, training, and guidance in daily living skills. Programming and service delivery is guided by the principles of recovery to assist individuals to achieve life goals. The Young Adult Learning Environment (YALE) provides additional educational, vocational, and social supports to better meet the needs of younger individuals 18 to 22 years old. This site is located in Bryn Mawr.

Positive Resolutions, within Montgomery County Residentials, is dedicated to enriching the lives of mentally ill adults by focusing on reducing the barriers to reaching their fullest potential and establishing supportive partnerships in recovery-oriented settings. Positive Resolutions is designed to support optimal personal, spiritual and social growth by facilitating activities that focus on skill development, wellness, and independence in residential settings. The sites for this position are in Norristown and Conshohocken.

Direct Duties :

Program Development and Monitoring :

• Maintain up-to-date knowledge and implementation of RHD programs.
• Be familiar with all program-related regulations from funders, community support services, government licensing agencies, and other related resources.
• Build and enhance relationships with RHD program staff and outside providers to coordinate care.
• Maintain a tracking system to identify the need for comprehensive treatment plans.
• Interface with in-house and external partner providers through monthly / quarterly off-site visits.
• Staff development and supervision :

Provide direct supervision to Care Coordinators and Resource Coordinators.
