The Department of Pharmacy Practice at the University of Illinois Chicago Retzky College of Pharmacy, is looking for a Clinical Faculty (Open Rank) / Clinical Pharmacist to join our Telemedicine Diabetes clinic.

The ideal candidate will be responsible for providing pharmaceutical care to patients in the area of diabetes management.

The pharmacist will join a team of physicians and advanced practice nurses to provide care to patients in an Illinois Department of Corrections (IDOC) facility via a telehealth model.

The pharmacist will work from the University of Illinois Hospital & Health Sciences System (UI Health) campus located approximately 2 miles west of downtown Chicago.

The hospital is a 450-bed academic institution located approximately 2 miles west of downtown Chicago. with 21 on-site outpatient clinics and 11 FQHCs.

The hospital and clinics serve an urban patient population.

The Retzky College of Pharmacy, with campuses in Chicago and Rockford, is the #1 college of pharmacy in the state of Illinois and #15 in total research grant funding according to the American Association of Colleges of Pharmacy (AACP).

The clinical pharmacist will be assigned to the IDOC diabetes telemedicine clinic 4 days per week with 1 office day per week.

The hours will be Monday - Friday during the day. The clinical pharmacist will work under a hospital approved protocol to participate directly in the assessment and management of patients with diabetes.

The pharmacist will recommend and implement an evidence-based care plan in collaboration with the treating credentialed practitioner, resident or fellow;

initiate, modify, or discontinue drug therapy for treatment of diabetes to achieve therapeutic goals; and order laboratory tests to assist in the management of diabetes.

The candidate will also be responsible for nurse, physician, and pharmacy staff development; coordinating and conducting quality initiative projects and developing collaborative treatment and research protocols.

The academic responsibilities of the clinical faculty member (Clinical Assistant / Associate / Full Professor) include the provision of didactic lectures and precepting entry level Doctor of Pharmacy students, PGY1 residents and PGY2 residents.

Scholarly activities, including presentations and publications, are an expectation as well. Academic rank commensurate with experience.

Minimum Qualifications :

Graduate of an ACPE accredited College of Pharmacy PharmD program

Candidate must have completed a clinical residency and / or fellowship or have 2-3 years equivalent experience for practice in a clinical setting and / or education / experiential education.

PGY 1 Residency or 2-3 years clinical pharmacy experience

Licensed, or eligible for licensure as a professional pharmacist in Illinois as of start date

Candidate must possess knowledge and skills such that they qualify for a non-tenured clinical faculty position with the

Department of Pharmacy Practice at the UIC College of Pharmacy

Preferred Qualifications

PGY2 Ambulatory Care residency or 2-3 years of equivalent clinical experience is preferred

Board certification is recommended
