WORK SCHEDULE* 40 Hours / Week* Day Shift, Monday-Friday* Hybrid schedule optional : M-W onsite / Th-F remote optionalPOSITION HIGHLIGHTS* Provide direct Behavioral Health care coordination and clinical services* Support and closely coordinate mental health care and psychosocial resource needs for patients in the UWMC Outpatient Psychiatry Clinic* Responsible for providing full range of social work services including clinical referral triage, care coordination, crisis therapy interventions, and resource navigation* Be part of a dedicate interdisciplinary team of psychiatrists, psychologists, and other social work providers for collaboration of care and improvement of patient wellbeing* Engage in various clinical support, research, and training opportunities within clinic for professional growthPRIMARY JOB RESPONSIBILITIESCare Coordination* Psychosocial needs assessment and resource coordination* Manage Social Work Referral Pool for resource requests, specifically SDOH and MH navigation* Facilitate care internal and external to UW Medicine with other community behavioral health programs for bridging to services and higher level of care* Provide patient education and assist in engagement in community resources / programs to meet psychosocial needs* Coverage of Employee Mental Health Services (EMHS) referrals as neededClinical Care* Referral triage for clinical treatment recommendations* Brief crisis therapy-interventions* Assessment of symptoms using validated screeners such as PHQ9, GAD7, ASQ, C-SSRS, and AUDIT* Caseload support for other clinical staff (social work, psychologists, psychiatrists)* Document patient encounters in the EMR to standards set by Compliance for billable servicesREQUIRED POSITION QUALIFICATIONSA Master of Social Work Degree from a program accredited by the Council on Social Work Education AND one year of full-time social work experience in a health care setting or equivalent.
