We have an exciting opportunity to join our team as a Home Health Aide- Home Care- Brooklyn-Full Time, Day.
In this role, the successful candidate A member of the Home Health Agency team providing personal care services to patients of all ages in accordance with the established plan of treatment in their Homes under the direct supervision of a Home care nurse.
Job Responsibilities :
- Encourages maintenance of the patient's medication, supplies and equipment. Reports environmental or equipment safety hazards to home office, patient, and family, with suggestions for improvement.
- Supports and complies with all departmental and hospital policies and procedures.
- Maintains caseload, schedules and travels to clients homes
- Performs patient care techniques, which require manual dexterity, visual inspection, manual palpation, auditory (auscultation);
visual discrimination and physical movement of patients and equipment.
- Performs other duties as assigned
- Provides personal care andor assists patients of all ages according to their abilities and the plan of care established by the Home Care Nurse via the following activitites : hygienic care, light housekeeping, preparation of meals, feeding, exercises.
- Observes and communicates changes in patient's status or condition as directed by the Home Care Nurse. Sets priorities.
Completes patient referrals to NYU WUH HHA, patient care reports, forms, daily vouchers, and schedules.
Minimum Qualifications :
To qualify you must have a High School Diploma or Equivalent. Certificate of completion of a state-approved program offered forHomeHealth Aides.
Department ofHealth (DOH) approved training program. 1 - 2 years Experience working as aHomeHealth Aide (HHA) / Personal Care Aide in aHome Care orHospital setting.
Excellent communication (both oral and written), interpersonal, and organization skills. Ability to negotiate architectural barriers in patient sHomes, and work within the environment of care.
Ability to travel to patientsHomes under all weather conditions.
Qualified candidates must be able to effectively communicate with all levels of the organization.
