The Adult Home Health team deliverscare to patients in the community providing a wide range of skillednursing care. We are a comprehensive team comprised of RNs PhysicalTherapists LPNs LNAs SLPs Social Workers and administrative supportall collaborating together to support patients to remain in livingin the setting of their choice theirhome.
POSITIONSUMMARY :
To provide a widerange of basic physical therapy services in a home care settingunder close clinicalsupervision.
MINIMUMQUALIFICATION STANDARDS :
Bachelors degree or higher in physical therapy
Two yearsor more general PT experience preferred; or a year of experience atthe PT I level at the University of Vermont Health Network HomeHealth & Hospice with a strong job performancerecord.
