The HOPE Care Manager is passionate, experienced and knowledgeable about the needs of youth in state custody. The HOPE Care Manager helps coordinate care and integrated services for eligible members.

In addition, they promote timely access to physical examinations and medically necessary services. They have experience and expertise with the state's child welfare system and will collaborate with the State of Michigan to make sure children in state custody have access to medically necessary treatments through referrals to appropriate providers.

Primary Responsibilities :

Assess, plan and implement care strategies that are individualized for children and youth in a family-oriented context (e.

g., considering reunification and / or other permanency planning needs, needs for adoptive children, and needs for transition-age youth with alternative permanency plans) and directed toward the most appropriate, lease restrictive level of care

Identify, initiate, coordinate, and monitor referrals for psychosocial, behavioral, and medical services

Manage the care plan throughout the continuum of care as a single point of contact

  • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
  • Advocate for children, youth, and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team
  • Provide trauma-informed and disease / disorder specific education to members and caregivers
  • Coordinate with the child welfare and juvenile justice system on behalf of members and caregivers
  • Coordinate with additional services, including but not limited to, educational / educational advocacy, complex behavioral health (PRTF, TFC, etc.

Required Qualifications :

Current, unrestricted license in designated field, LMSW (Licensed Master Social Work) or LCSW (Licensed Clinical Social Work)

  • 2+ years of child welfare experience
  • 2+ years of working with pediatrics / children
  • 1+ years of experience with MS Office, including Word, Excel, and Outlook
  • Reside in State of Michigan
  • Reliable transportation and the ability to travel within assigned territory to meet with members and providers
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Preferred Qualifications :

  • Master of Social Work (MSW)
  • 2+ years of Clinical Experience
  • 1+ year(s) of community case management experience coordinating care for individuals with complex

needs and experience working with the foster care population

Background in Managed Care, especially in Michigan

Experience working in team-based care

CCM certification
