Here's What You Need

Bachelor's Degree, Bachelor of Science in Nursing (BSN) is required. (CLIAFE) Must comply with CLIA regulations regarding foreign education equivalency required.

3 years of RN experience in an acute care hospital is required.

1 year of discharge planning and / or care management is highly preferred.

Must hold an active RN license - Registered Nurse license upon hire is required.

CPR - Cardiopulmonary Resuscitation is required upon hire.

ACM - Accredited Case Manager upon hire is preferred OR CCM - Commission for Case Manager Certification or ACM - American Case Management Certification upon hire is preferred.

What You Will Do

Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner. In addition to the required qualifications, a successful RN will :

Serves as a point of contact for all identified stakeholders. Attempts to schedule PCP, specialist or clinic follow up appointments for patients.

Promotes discussion and assists in the identification of a primary care physician (PCP) for patients without a PCP.
