JOB SUMMARY

AtHouston Methodist the Physical Therapist (PT) position isresponsible for assessing patients and developing the therapy planof care.

This position provides education to patients patientsfamily members and / or caregivers and collaborates with other healthcare professionals as needed for problem resolution and / orachievement of identified patient specific goals.

As a member ofthe patient care team the PT position provides direct patient careand has responsibility to deliver exceptional patient care withadherence to safety standards and guidelines to ensure optimalpatient safety customer service and optimum patientoutcomes.

Performs complete andcomprehensive evaluation of patients to identify patients level offunction and to determine the treatment plan.

Develops the plan ofcare based on evaluation findings and implements the physicaltherapy treatment plan. Practices in a manner that is congruentwith cultural diversity and inclusion principles.(EF)

  • Provides education to patients patientsfamily members and / or caregivers based on learning needs.Consistently evaluates the patients comprehension and adaptsteaching methods accordingly. (EF)
  • Collaboratesand communicates with the healthcare team modifications to patientstreatment plan progress and discharge planning for achievement ofidentified patient specific goals.

Advocates on patient / familysbehalf to identify and resolve clinical and ethical concernsutilizing appropriate resources.(EF)

Contributes towards improvement ofdepartment score for patient satisfaction through peertopeeraccountability to service standards.(EF)

  • Assesses patients regularlymonitors progress towards treatment goals modifies goals and planor program as indicated. Recognizes complications / limitationsassociated with certain procedures and collaborates with peers toaddresses for optimum outcomes. (EF)
  • Providesinput of physical therapyrelated activities completed byinterprofessional health care team and if necessary adjusts theplan to improve quality of outcome and maintain the safety of thepatient. (EF)
  • Identifies opportunities forcorrective action and process improvement. Identifies areas ofpractice improvements with researchbased evidence to achievepatient safety outcomes supported by accurate documentation.

Contributes to meeting department and hospital targets for qualityand safety. (EF)

Maintains accurate and completedocumentation of patient education procedures performed in a timelymanner in the patient record.

Enters accurate charges for servicesprovided as applicable. (EF)

Uses resourcesefficiently to maximize utilization of time and equipment.Selfmotivated to independently manage time effectively minimizingincidental overtime and prioritize daily tasks assisting coworkersas needed.

Meets labor productivity standards by effectivelymanaging established patient load assignments.(EF)

Assists higher level PTs with programdevelopment and / or clinical competencies. Ensures own careerdiscussions occur with appropriate management.

o Texas Physical Therapistlicensure or eligible for temporary Texas Physical Therapy licensewith the Texas Board of Examiners must obtain licensure within 90days of employment.
