Role : Center Manager / Director (Head of Operations), Senior Day Health Center Organization : Edenbridge PACE West Baltimore Location : onsite in West Baltimore, Maryland Start date : Oct 2024 Status : Full-time Mission : To allow frail elderly people to age in the location of their choosing and continue to lead connected, meaningful lives The Center Manager / Director role offers a unique opportunity to help build a PACE (Program of All-Inclusive Care for the Elderly) program in West Baltimore within the framework of a supportive, innovative, and expert leadership team and company.

Together with our coalition of strong partners within the community, the Center Manager will work closely with the Executive Director to lead West Baltimore PACE to have a transformative impact on a particularly under-served community.

Seven core principles drive our approach to care. These include : Create and Sustain Meaningful Lives : Our model aims to help elders create and sustain meaningful lives from social relationships, spirituality, cultural life, work, or service.

We support elders with cognitive and physical disability to sustain meaning in their daily lives. We form close community partnerships to help elders engage outside our centers.

Respect the Right to Independence : Edenbridge aims always to follow our participants’ wishes. Wherever possible, participants maintain control over all decisions from medical procedures to what to eat for dinner.

Pay Attention to What Is Working Well : Our goal is to multiply the days each participant is glad to be alive. We encourage participants’ positive experiences of daily life, supporting the things that matter most to them.

Provide the Right Amount of Medical Care : Elders often have many diagnoses, medications and specialists, involving complex care decisions.

We help our participants understand their choices. We deliver thoughtful, appropriate and personalized care, doing what our participants want nothing more and nothing less.

Guide All the Care Provided : We always follow and guide our participants’ care in their homes, at our center, in the hospital, and everywhere.

Live with Our Communities, Not Just in Them : We work in communities where aging in place is valued and integrated care is a shared mission.

We build deep relationships with community organizations ( see video ) beginning long before we open our doors. We aim to reach beyond the walls of the center to bring community members in and participants out for person-to-person connection .

Employ Technology Thoughtfully, Not Aggressively : We use technology because it can enrich the lives of participants, improving care and connecting family members with their loved ones and their care providers.

But we use technology only to complement never replace direct human interaction. To help us achieve this mission, we are looking for an experienced healthcare clinic operations leader to develop, lead, implement, organize, and monitor the PACE Health Center program.

  • This position will report to the Executive Director. Main Responsibilities : Ensure the smooth operation of the PACE program by overseeing the daily functioning of the PACE center Supervise and train day center staff Facilitate interdisciplinary (IDT) team leadership meetings;
  • provide leadership for effective care planning and problem solving for quality participant service. Develop, implement, and manage participant activities Manage the coordination of daily care schedules Ensure the effective operation of the Center as the focal point of the Edenbridge PACE program model to meet participant needs;
  • Effectively communicate and coordinate with all Edenbridge PACE program departments. Supervise interdisciplinary team staff including home care coordinator, clinic nurse manager, social workers, activities coordinator, physical therapist, occupational therapists, dietician, transportation coordinator, and personal care attendants;
  • and other center staff as assigned, including center receptionist. Assist in the development of organizational staffing structure and position descriptions for Center staff;

assists in the selection and orientation of employees. Assist with development, oversight, implementation, and adherence to policies, procedures, and standards for all services, in consultation with other management staff.

Assist in the development and administration of the Edenbridge PACE program budget, in cooperation with the Executive Director Monitor and analyze the Edenbridge PACE program performance trends in utilization, financial, and participant outcomes Responsible for assisting to meet target enrollment and quality improvement plan goals.

Participate in and contribute to the Edenbridge PACE program management team. Ensure adherence to all regulations, both federal and state, which govern the operations of an Edenbridge PACE organization and a licensed adult day care center.

Communicate with participants and their families, contracted providers, and the public in regard to issues related to the routine operation of the center Additional responsibilities as needed;

For our Center Manager role, we are looking for the following qualifications : Bachelor’s degree; Master’s or equivalent work experience preferred 5+ years experience with healthcare delivery operations and management Experience serving low-income, diverse and marginalized communities Experience with development and implementation of programs for the elderly population preferred Direct PACE experience a huge plus;
