Today we care for patients who need skilled home care and hospice services. You won’t find our brand in many places because we partner with health systems, jointly running home health and hospice agencies that use their brand.
This is part of our strategy. By being part of the health system team, we can ensure each patient has a well-coordinated care plan that remains consistent whether the patient is seeing their primary care physician, receiving treatment in a hospital, or under our care in the home.
HOW YOU'LL WORK :
You’ll help direct the activities of assigned community staff and provide relationship development and management related support to market / agency / program management to ensure market growth and profit targets are achieved or exceeded.
MAJOR AREAS OF RESPONSIBILITY :
Manage and oversee overall community growth and strategic plan for dedicated agencies while also managing daily assigned accounts
Work with region and agency leadership to implement appropriate growth and conversion strategies and prioritize key customers in accordance with market strategic growth plan
Collaborate with clinical team members on complex patient cases to provide for the best clinical outcome
Identifies future patients and determines home care eligibility
Reviews patient insurances and medical documentation to determine home care appropriateness
Coordinates health care services as ordered by the attending physician
Assists hospital / facility personnel in the discharge planning process
Work with leadership to develop and execute market-specific annual strategic plan / budget and prepare quarterly admissions updates to ensure achievement of established growth targets and compliance with established budgetary constraints
Maintain detailed territory list, prioritizing accounts in accordance with the market / territory growth plan. Maintain detailed territory management systems records as required
Act as a liaison between referral sources and company staff to provide information on company services, resolve problems and maintain positive customer relations
Maintain market awareness and prepare competitive updates
ay have responsibilities as a mentor, as designated by VP Partnership and Community
Participate in special projects and perform other duties as assigned

Proficiency in Microsoft Excel, Access, Word, and Outlook and the ability to learn new systems.
REQUIREMENTS :
Bachelor’s Degree or equivalent combination of education, training and experience
Three to five (3-5) years of market growth and maintenance experience, preferably in home care or in the medical field or a combination of relationship / clinical delivery
Registered Nurse, Licensed Practical Nurse, Physical Therapist, Physical Therapist Assistant, or Licensed Social Worker is preferred in the state of services
Ability to travel
Valid Driver’s License and Auto Insurance
Capable of all physical demands
