Our patients are in Rockland County, NY. and Orange County, NY.
Home Health Aides (HHAs) provide critical professional patient personal care on behalf of United Hospice (UH) as the end-of-life care provider of choice within the healthcare community.
The HHA is a key member of our patient care team and serves as an integral liaison between the patient, their family, and RN Case Manager with a broad range of vital health and personal care responsibilities as outlined in the Plan of Care.
We are currently Looking for HHAs for the communities of Orange County
Pay Rate : $18 / hr
Responsibilities of HHA will include, though not be limited to :
Follows the Plan of Care established by the hospice nurse.
Actively participates in the coordination of all aspects of patient care, in accordance with current professional standards and practice.
Performs health care procedures monitored by hospice nurse (example : reinforce clean dressings, use of bedpan, check urinary catheter, take vital signs, irrigate urinary catheter etc.)
Provides for the physical care of the patient, as stated in the HHA care plan, (example : bathing, toileting, skincare, etc.).
Assists the patient with ambulation, transfers and turning and positioning as needed.
Turns and positions patients who are unable to assist.
Under the guidance and instruction of primary nurse or P.T, O.T. maintains a safe environment for patients which includes the proper use of equipment (i.
e. wheelchair, Hoyer lift).
Assists the nurse by reporting changes in the patient's condition or needs.
Helps patient in performing exercises prescribed by appropriate professional personnel.
Assists community patients in taking self-administered medication prescribed by physician.
May plan and prepare meals for patient, and feeds patient when necessary.
Performs a variety of light housekeeping tasks when necessary which are essential to the patient's health care.
Performs washing of patients' clothes as needed.
Community HHAs keeps an up-to-date record of activities and the patient's condition to be submitted to the hospice nurse for incorporation into the patient's records.
Community HHAs, provide temporary respite for family caregivers during assigned work times.
Community Aides may do errands as needed.
Attends team meetings as needed.
Attends twelve (12) hours of documented training annually that may come from any acceptable source, (i.e. documentation from other employment).
Participates in and remains compliant with, all required QAPI programs and hospice-sponsored in-service training.
Incorporates the organization’s commitment to customer service and continuous quality improvement into daily work habits. This includes :
Promotes positive co-worker and teamwork relationships.
Embraces our shared core values, code of conduct and mission as set forth by the Board of Directors and administration of the agency.
Communicate all pertinent issues to the Assistant Director of Nursing or Director of Clinical Services.
At the direction of the Assistant Director of Nursing or Director of Clinical Services, accepts additional responsibilities as assigned.
Qualifications :
Completion of a New York State Health Department-approved home health aide training program and maintain that certification by annually completing 12 hours of approved continuing education.
Certification as a home health aide and successful completion of ongoing competency testing.
Working knowledge of basic first aid.
Basic understanding and commitment to the hospice philosophy of care.
Ability to work as a team member and use supervision effectively.
Ability to maintain a high level of confidentiality concerning patient and family information.
Ability to relate to a diverse population and workforce in a personable and effective manner.
Must possess computer and email skills.
Additional Requirements :
Community HHAs require daily access to an automobile. A valid Driver’s License and ability to drive and driving record acceptable for agency insurance coverage are required.
