- Prime Fingerprint required for onboarding
- American Heart Association or Red Cross BLS and ACLS certifications required
- Primary Source Verification of professional license / certification required
- NIHSS certification required
- Prime Basic Pharmacology Exam, score 80% or higher
- Prime Basic Rhythm Interpretation Screening, score 80% or higher
- Prime Orientation Packet required
- COVID-19 Vaccine required
- MMR Proof of 2 Vaccines or Positive Titer showing immunity
- Annual Respirator Fit Test requirement
- Hepatitis B vaccination required
- Flu Vaccine or declination required Oct 1-May 31 with specific facility exceptions
- TB Test required
- Driver's License / State ID / Passport required
- Varicella Vaccination / titer showing immunity
- Tdap requirement with facility-specific declination policies
- Physical / Health Statement requirement
- Prime OIG, Prime SAM / EPLS
- Prime Criminal Background Check policies
- Prime HR Attestation required after all other credentials are uploaded
- Prime Badge Photo, JPEG only photo
- Prime Form I-9 (Employment eligibility verification) required
