In this role you'll be responsibile for adequately assessing our residents' needs and creating personalized care plans based on the assessments.

WHAT WE'RE LOOKING FOR Here's what you'll do when you join us : Manage the Resident Assessment Instrument (RAI) process to assess residents' individual needs to create a unique care plan based on your findings.

Promote cross-functional communication to ensure care plans are followed. Ensure appropriate reimbursement for resident care services.

Be well versed with current federal, state, and local standards, guidelines and regulations.
