Physician - Senior Focused Care - Prime 65+ by Mercy - Hazelwood, MO

Mercy is recruiting board eligible or board certified Physicians with a specific interest in building relationships, focusing on wellness, changing lives, and making a difference for seniors.

This is an outpatient-only position with an established patient panel size of 600 patients on Medicare and Medicare Advantage.

Practice is located in Hazelwood, MO.

• Value-based care - time with patients, relationship building, quality incentives.
• Dedicated panel RN for telephone encounters, triage, and in basket support
• In-person, virtual, or telephonic encounters - annual comprehensive visits, transitional care, and acute / urgent visits

• 30 minute - 1 hour appointment slots for all patients - adequate time without rush
• Manage a patient panel size of 600 or 1,000 with APP support
• No holidays, weekends, or evenings
