The Readmissions Reduction Clinic Post-Acute Navigator is pivotal in addressing the needs of a high-risk population and involves a combination of clinical and administrative responsibilities.
Significantly contributes to reducing hospital readmissions by actively managing patients and coordinating with healthcare providers.
Fosters patient / family self-management and compliance with individualized plan of care via post-acute phone calls.
Education
Bachelors Degree : in Nursing (Required)
Masters Degree : in Nursing (Preferred)
Combination of relevant education and experience may be considered in lieu of degree.
Experience
2 Years nursing experience (Required)
2 Years experience in oncology nursing (Preferred)
2 Years navigation experience (Preferred)
License / Certifications
RN-LIC - PA Registered Nurse License (Required)
CCM - Certified Case Manager (Preferred)
