Jordan Health is now seeking aTeam Care RN Manager, Pediatricswho, under the guidance of the Director of Clinical Operations, is primarily responsible for working collaboratively to identify and proactively manage the care needs of patients within the family medicine practice setting.

The RN provides assessment, care coordination, advocacy and coaching for patients.

Requirements

The Team Care RN Care Manager, Pediatrics will ensure to :

Education And Experience Required :

1 year of nursing experience required. Preferred experience in urgent care, emergency department, pediatrics, or OB / GYN.

Licenses And Certifications :

Valid RN licensure in NYS with no negative actions.

Current Provider BLS / CPR through the American Heart Association.

Infectious Control certification.

Special Skills, Knowledge Required :

Strong computer skills.

Excellent customer service skills both phone and in person.

Ability to work well with others in a multi-disciplinary team.

Ability to organize and multi-task.
