About The Position

BHS Hospital Based - Social Worker (No local candidates within 50 miles.)

Conduct psychosocial assessments of patients and families.

Provide counseling, emotional support, and crisis intervention.

Develop care plans and coordinate with medical teams.

Assist with discharge planning and community resource referrals.

Advocate for patient needs and educate on care options.

Document interventions and ensure compliance with hospital policies.

Min of 2 years of experience within specialty - Hospital Based experience.

Required : BLS & LCSW required in the state of GA.
