The Advanced Practice Provider is primarily responsible for providing health care services in the long-term care environment, including assessing, diagnosing, prescribing, treating, and educating residents.
The Advanced Practice Provider works closely with physicians and other clinicians in a team approach to resident care. The Advanced Practice Provider is a highly visible position that is always responsible for creating a positive impression with residents, families, facility staff members, physicians and other visitors he / she encounters, both in person and on the phone.
In partnership with VOA, St. Elizabeth is excited to bring a PACE program to the Northern Kentucky Community.
Programs of All-Inclusive Care for the Elderly (PACE). PACE provides comprehensive services to individuals aged 55 and older who need nursing facility level of care but prefer to continue living at home.
Enrollees receive services through a PACE organization which works to meet all preventive care, acute care, long-term care and social needs.
Responsibilities
Oversees community care for PACE enrollees.
Completes CMS regulatory visits face to face visit requirements in collaboration with the attending physician.
Provides clinical information to the interdisciplinary team as well as the physician to ensure appropriate care and collaboration.
PACE coverage area is 8 counties in the Northern Kentucky region.
Coordinates care with physician, family members and community partners when PACE enrollees plan of care is outside standardized practice and protocols.
On call coverage rotation.
Qualifications
- Knowledge of advanced practice in geriatrics including anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial / family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities for aging population.
- Knowledge and discrete understanding of environmental-specific problems, geriatric patient stress, delirium, dementia, and depression.
- Knowledge of medical equipment and instruments used to administer patient care.
- Knowledge of common safety hazards and precautions to establish / maintain a safe work environment for the aging population.
- Knowledge of health promotions, health risk identification, and patient education principles and techniques.
- Knowledge of community, professional and education resources.
- Must be adaptable to working in a variety of long-term care facilities.
- Excellent observation, verbal, and written communication skills, problem solving skills, mathematical skills, and nursing skills.
- Knowledge of considerable autonomy, and collaboration with multidisciplinary teams, physicians / Medical Director and others involved in resident's care directly and indirectly.
- Must coordinate care with the facility staff, outreach areas (as applicable), family members, and work collaboratively with all physicians involved with resident's care directly and indirectly.
- Requires travel in own vehicle as needed.
- One (1) year nurse practitioner experience.
- One (1) year geriatric nursing experience.
- Prior PACE, clinic, palliative and or geriatric experience preferred.
- You must be able to obtain a state license in KY for your scope of practice.
