About the Position
The Post-Acute Care Transitions Program is seeking a physician to join our established team. You should have a current CA medical license, DEA, and eligibility to participate in Medi-Cal / Medicare.
The successful candidate will work closely with our robust interdisciplinary team to provide clinical care to our diverse, fragile patients, as well as coordinate transfers from the hospital to the contracted SNF and facilitate safe discharges to the community.
There is also an opportunity for QI projects for the division as well as teaching Internal Medicine residents. This position offers full-time employment to work half time in Post-Acute Care Transitions and half time in the Primary Care clinic.
