PER DIEM OCCUPATIONAL THERAPIST

• Physicians Choice Home Health *is hiring a per diem Occupational Therapist to provide care to patients in their private residences, group homes, assisted living facilities, and independent living facilities.
• We also need coverage for Pahrump, NV area.*
• This position is highly flexible and allows the Occupational Therapist to make his or her own schedule.*
• QUALIFICATIONS AND EXPERIENCE* :
• Graduate of an accredited Occupational Therapy program.
• Licensed with the State Board of Occupational Therapy and registered with American Occupational Therapy Association.
• At least one year of experience as an Occupational Therapist; home health experience is preferred.
• Current CPR certification.
• Negative TB test.
• Vaccination against COVID-19, preferred but not mandatory.
• Current driver’s license and automobile insurance.
• ACCOUNTABILITY : *

Reports directly to Home Health Director of Nursing.

• DUTIES AND RESPONSIBILITIES* :
• Provides Occupational Therapy services to clients according to a written physician’s plan of care.
• Develops a treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other agency professions.
• May teach new skills or retrain client in once familiar daily activities that have been lost due to illness or injury, in accordance with company policy.
• May fabricate splints and instruct client in use of various types of adaptive equipment to improve function.
• May train client in the use of prosthetic and / or orthotic devices.
• Secures and maintains medical equipment / assistive living devices relevant to client’s needs / conditions.
• Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the client’s condition.

Submits these records in accordance with company policy.

• Maintains contact / communication with other personnel involved in the client’s care to promote coordinated, efficient care.
• Identifies client and family / caregiver needs for other home health services and refers, as necessary.
• Supervises Certified Occupational Therapy Assistants in responsibilities.
• Participates in performance improvement activities as well as survey and accreditation process.
• Maintains professionalism while representing PCHHC as an employee.
• Adheres to infection control policies and guidelines.
