Position Details :

• Job Title : Home Health RN
• Location : Carle Place, NY
• Schedule : Full-time, 40 hours per week (Monday-Friday, 8 : 00 AM - 5 : 00 PM)
• On-Call : One weekend per month
• Patient Population : Adult and Geriatric
• Productivity : 25-30 units per week
• Mileage Reimbursement : $0.48 per mile (visit to visit)
• Benefits : Medical, Dental, Vision, 401(k) after 90 days
• Pay : Negotiable based on territory budget and candidate experience, $50-$60 per hour, common range negotiated

Key Responsibilities :

• Patient Care : Conduct comprehensive assessments and provide skilled nursing care to patients in their homes, ensuring care plans are executed with precision.
• OASIS Documentation : Complete OASIS (Outcome and Assessment Information Set) documentation accurately to ensure proper patient care planning and compliance with regulatory standards.
• Homecare Homebase : Utilize Homecare Homebase (HCHB) software to document patient visits, manage care plans, and communicate with the healthcare team.
• Coordination : Collaborate with physicians, physical therapists, and other healthcare professionals to deliver coordinated, patient-centered care.
• On-Call Support : Provide weekend on-call support on a rotational basis, addressing urgent patient needs.
• Patient Education : Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote optimal health.

Requirements :

• Active New YorkRegistered Nurse (RN)License
• Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross
• Minimum of 1 year of experience as an RN in a home health setting
• Proficiency in OASIS and Homecare Homebase (HCHB) software required
• Reliable transportation and a valid drivers license
• Ability to complete Joint Commission onboarding standards
