Programs in Assertive Community Treatment (PACT) is an evidence-based outreach program to help people with serious mental illness integrate into the community.
Bright Harbor Healthcare provides PACT services to more than 160 people in all of Ocean County. The PACT program is a multi-disciplinary team for persons with a serious and persistent mental illness.
A team consists of a psychiatrist, advanced practice nurse, nurses, clinicians, co-occurring specialists, vocational specialists, mental health specialists, and peer mental health specialists.

Responsibilities :
- Participate in multi-disciplinary treatment team for collaboration of client care
- Provide case management services including consumer advocacy
- Assists consumers with obtaining basic needs and activities of daily living through modeling, direct assistance with ADLs and coaching
- Provides on-going assessment of client’s emotional well being, clinical symptomology, behavioral changes and medication compliance
- Provides psychosocial education for consumers and family members on mental illness, symptom management, medication compliance and life skills
Requirements
- Graduation from an accredited school of nursing
- One year experience working with consumers with mental illness
- Current license to practice as an LPN in New Jersey
- Valid NJ drivers license with less than 6 points
