Zarephath is a behavioral health agency that works with children and families experiencing behavioral health challenges. Case Managers help clients birth to 17 with a behavior health diagnosis to gain access to community services.

Case Managers work with the guardians / caregivers and other agency staff to provide well rounded care plan for each client.

Responsibilities and Duties Shifts : Monday Friday - 8 : 30am-5pm Assessing clients’ physical and mental wellness, needs, preferences, and abilities Develop tailored care plans.

Working with clients, family and friend support networks and health care professionals to put care plans in place. Recording clients’ progress including charting referrals, home visits and other notable interactions.

Evaluating clients’ progress periodically and adjusting as needed to improve outcomes. Following up with discharged clients to ensure they were satisfied with services, and they are still in good physical and mental health.

Qualifications and Skills - Must be 21 years of age - Minimum 2-year experience working in behavioral health field working with youth and families.

  • Possess a valid Arizona driver's license and be insurable under the agency's automobile policy. - Possess a valid Arizona Fingerprint Clearance Card or be able to obtain one.
  • Must be computer literate with knowledge of Outlook and Microsoft Office programs (i.e. Word, Excel) - Must be fluent in Spanish and English Pay Rate : $36,000-$40,000 per year. Powered by JazzHR
