Husky Senior Care is an in-home care agency dedicated to providing quality service to all our clients. We are currently seeking a highly motivated and compassionate Client Care Coordinator to join our team.
As a Client Care Coordinator, you will be responsible for coordinating the delivery of healthcare services to our clients, ensuring their safety, comfort, and well-being.
This is a full-time position with competitive pay and benefits. Expected hours will be Tuesday-Saturday from 10am-6pm, however this position will require flexibility and the ability to adapt to changing tasks and responsibilities at a moments notice.
This means your schedule may change on a weekly or daily basis, but you are guaranteed 40 hours per week. The Client Care Coordinator is paid at $28 / hour.
The Client Care Coordinator’s job is to ensure we will always fulfill our commitments to our clients and caregivers. Care Coordinators will manage a caseload of clients to make sure they are getting top notch care.
They will fill in the gap in coverage on shifts, or act as the starting caregiver when onboarding new clients. This position will require flexibility and the ability to adapt to changing tasks and responsibilities at a moments notice.
We are seeking a female Client Care Coordinator to provide personal care services to female clients who have a preference for same-sex caregivers.
This requirement is a Bona Fide Occupational Qualification (BFOQ) necessary for the role.
Responsibilities :
- Coordinate and manage the delivery of healthcare services to clients.
- Help manage up to 15 clients and covering shifts for start-up clients as well as emergency shift coverage (callouts).
- Managing client expectations towards caregiver and being liaison between the client, caregiver, and headquarters
- Assist in client assessments and develop care plans.
- Train caregivers, ensuring they are providing high-quality care.
- Support our caregivers by providing support visits, training, advice, and correction as needed.
- Communicate with clients, families, and healthcare professionals to ensure continuity of care.
- Perform regular client check-ins and monitor their progress.
- Ensure compliance with healthcare regulations and agency policies.
- Travel to multiple clients as needed.
- Problem solves in special customer care circumstances, including change in condition, emergency start-up, discharge from hospitals, hospice start-up, and setting up nurse delegation.
Requirements :
- Associates Degree or some college.
- Microsoft Office experience.
- at least 2 years of caregiving experience.
- CNA or HCA license.
- Nurse Delegation certificate.
- Valid Driver's License and willingness to travel to multiple clients in the same day.
- Must be okay with pets.
