Nurse Practitioner/Physician Assistant - Home Based Primary Care

Work Schedule :

80% FTE, 8 hour days, 4 days a week, core hours of 8 : 00am to 5 : 00pm. Hours may vary based on the needs of the department.

As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients.

We are seeking a Nurse Practitioner to :

• Work in a new and innovative Home-Based Primary Care program working with older adults that are homebound.
• Collaborate with a focused interdisciplinary team to care for patients.
• Conduct comprehensive or episodic health history and physical assessment of patients / families with acute, chronic and / or potential health problems in the patient's home.
• Provide appropriate direction to program and clinic staff regarding patient care issues.

Education : Minimum -

Minimum -

If Nurse Practitioner :

Master's or Doctorate's degree from accredited Nursing Program.

If Physician Assistant :

Bachelor's or Master's degree from accredited PA Program

Licenses and Certifications :

Minimum -

If Nurse Practitioner* :

• Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND valid WI Drivers license and reliable vehicle
• Active board certification as a NP in area of clinical practice. These may include the following : Adult;; Adult / Gerontology Primary Care; Family; Gerontology;

If Physician Assistant* :

• Active PA license, including prescriptive authority, in state(s) of practice AND
• PA board certification

All Nurse Practitioner and Physician Assistant applicants must have* :

Active DEA license or ability to apply for such license prior to or upon hire

Active license or eligibility to obtain active licensure in state of practice. This may include licensing in Wisconsin and other state(s) of practice.
