Job Title : Registered Nurse I Job Specialty : Home Health Job Duration : 13 weeks Shift : Day Shift, Monday to Friday, 8 : 00 AM to 5 : 00 PM Guaranteed Hours : Not guaranteed Experience : Minimum of 1 full year of Adult Home Health or Hospice experience License : Must have a current and valid nursing license Certifications : CPR and BLS required Must-Have : Proficiency in OASIS and experience with HomeCare HomeBase (HCHB) EMR within the last year- Reliable transportation, a current driver's license, and liability insurance- Comfort with on-call duties and weekend rotations- Willingness to travel up to 75 miles from the assigned care center Job Description : Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides, Licensed Practical Nurses, and Licensed Vocational Nurses.- Perform home visits and periodic office meetings.
- Manage a caseload with an average of 5-8 patients per day.- Complete documentation within 24 hours of patient appointments.
- Meet weekly productivity expectations of 30 points, equating to approximately 40 hours per week.- Handle various types of cases, including Start of Care, Wound Care, and Follow-up Visits.
- Travel between patient homes, covering approximately 150-200 miles per week, with mileage reimbursed at the IRS standard rate.
- Engage in on-call rotations and weekend assignments as needed.- Accept every patient assigned, regardless of travel distance between appointments.
