DEPARTMENT SUMMARY : The Los Angeles LGBT Center’s Department of Health Services is one of the nation’s largest and most experienced providers of LGBTQ-specialized primary care and related services.
Our mission to help LGBTQ people lead healthier, happier lives is supported by our comprehensive array of client-centered services including primary care, mental health, psychiatry, sexual health, pharmacy, case management, nutrition, clinical research, housing services, and a food pantry.
The Center is one of a handful of Federally Qualified Health Centers in the nation specializing in serving LGBTQ people.
JOB SUMMARY : Under the supervision of the Director of Case Management Services, the Manager of Nurse Case Management Services provides direct supervision and oversight of the RN Care Managers, the Assistant Manager of Nurse Case Management Services, and the Audre Lorde Health Program (ALHP) RN Manager.
The Manager of Nurse Case Management Services ensures that Case Management Services operates with-in industry standards and meets all contractual requirements and guidelines.
of Role ESSENTIAL FUNCTIONS :
Program Planning and Management
- Provides overall direction and guidance for Case Management Services.
- Provides direct supervision and guidance to the RN Care Managers, the Assistant Manager of Nurse Case Management Services, and the Audre Lorde Health Program (ALHP) RN Manager.
- Provides indirect supervision and guidance to the LVN Case Managers.
- Provides direct supervision of the Medical Care Coordination (MCC) and other medical case management programs.
- Works closely with the Director of Case Management Services, Director of Nursing, MCC Teams, Medical Providers, HS Managers, and staff to ensure that Case Management and Specialty Care Services meet patient care and regulatory requirements.
Quality Management
- Provides Quality Management direction, support, and oversight for Case Management activities in all HS divisions and other LA LGBT Center areas as appropriate.
- Actively participates in Quality Management (QM) Committee and ensures that regular monitors are conducted and reported on.
- Leads and / or participates in various special projects teams to address areas of concern.
Clinical Compliance
- Develops and maintains current Policy & Procedure (P&P) for Case Management services and ensures that they comply with all regulatory and contract requirements.
- Actively participates in the Nursing Unit Level P&P Committee
- Conducts regular meetings with staff to ensure full understanding and compliance for all P&P, QM plans and risk management activities.
- Participates as needed in all program, licensure, and other external audits and in the preparation of Plans of Corrective Action.
Miscellaneous
- Participates in LA LGBT Center and HS meetings as required.
- Attends outside meetings and represents the organization as needed.
- Other duties as assigned.
Management Responsibilities (Budgetary & Direct Supervisory)
- Responsible for all human resources functions for direct reports including hiring, firing, and disciplinary action.
- Maintains appropriate documentation and ensures compliance with all LA LGBT Center policies and requirements.
- Ensures that staffing levels and coverage are adequate to meet patient care and regulatory requirements.
- Maintains systems to provide regular feedback and evaluation for employees.
- Works closely with management to maintain a positive patient care atmosphere in the clinic and throughout the organization.
- Develops and implements staff training. and develops programs to meet continuing education and contractual requirements to further increase knowledge in the provision of case management services, specialty care, and nursing / medical care.
- Develop and maintain a supervisory system that includes preparing monthly work schedules, staff call-in log, regular staff meetings, regular one-on-one meetings, a training program, performance documentation, and personnel files for residential programs.
- Recruit, hire, train, schedule and supervise staff, planning staff training in coordination with operations team.
- Work closely with the Youth Services Data and Evaluation Manager to ensure all staff are fully trained on operational databases (HMIS, Sharepoint, Clarity, Civi, MS TEAMS).
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.
Knowledgeable of and sensitive to the issues affecting LGBTQ youth, particularly those who have experienced homelessness, are at-risk, and / or may have substance abuse and mental health issues.
Sensitive to the needs of gay, lesbian, bi-sexual, and transgender youth from a wide range of backgrounds.
Demonstrated ability to work effectively with people of diverse races, ethnicities, nationalities, sexual orientations, gender identities, gender expression, socio-economic backgrounds, religions, ages, English-speaking abilities, immigration status, and physical abilities in a multicultural environment.
Minimum three years’ experience in direct patient care, preferably in an ambulatory care setting.
Minimum two years’ experience in Case Management.
Minimum two years’ increasingly responsible nursing management, experience preferably in out-patient setting.
Experience in care / management of HIV / AIDS and other infectious chronic diseases.
Basic knowledge of state / federal reporting requirements pertaining to infectious diseases.
Understanding of budget processes, quality assurance and improvement, distribution reports, DHSP, HRSA, FQHC, PCMH standards, and other regulatory standards including state and federal laws, rules, and regulations.
Strong communication, organizational, and interpersonal skills, with a high level of attention to detail.
Proficiency with Microsoft Office, Electronic Health Records, and other software systems.
Self-motivated and dedicated to initiating and completing complex tasks.
Ability to handle multiple tasks while maintaining attention to detail and strict confidentiality of medical information.
Ability to work well independently with minimum supervision.
Bilingual (English / Spanish) skills a plus.
TRAINING, EDUCATION, AND LICENSURE
Current California RN License
Current AHA CPR Certification
BSN or MSN preferred
Current Active ACRN Certification preferred
