Hamilton Center, Inc. Nurse, Registered- CCBHC MOBILE CRISIS UNIT Department : Crisis Services Division : Medical / 24-Hour Services Supervised By : Manager of Crisis Diversion Position Summary The CCBHC Mobile Crisis RN will be part of a 24-hour crisis stabilization hub that includes enhanced 24- hour crisis services and a mobile crisis unit that provides crisis stabilization services to the identified populations.
Provides and directs physical and mental health care of patients located primarily in the Crisis Diversion Center, but must be flexible to the needs of HCI including working in other areas of the organization such as Vigo County Outpatient or Inpatient Unit.
Functions as a part of the interdisciplinary treatment team, in which patient care is prescribed. Essential Duties / Responsibilities To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

- Demonstrates the ability to conduct and monitor the nursing process, which consists of assessment, planning, implementation and evaluation of nursing care.
- Conduct admission assessments and reassessments as indicated according to policy, procedures and standards of care.
- Follow admission and discharge procedures.
- Works closely with law enforcement and community agencies to provide immediate intervention aimed at reducing the risk of emergency room and psychiatric hospitalization.
- Functions as a member of The Mobile Crisis team consisting of Crisis Diversion Specialists, on-call therapists and medical team.
- After stabilization of the crisis situation, will assist in connecting the individual to appropriate level of care.
- Obtain and transcribe correctly the licensed independent practitioner’s (LIP) orders for admission to the inpatient unit.
- Participate as a member of the interdisciplinary team.
- Document in the patient's record according to policy and procedure.
- Assist in the identification of patient and family educational needs.
- Participate in the teaching of patient and family educational needs especially related to physical issues.
- Provide daily information at shift change and treatment review / discharge meetings.
- Participate in CQI activities as assigned.
- Complete safety plans and internal referrals for continued services after crisis has been stabilized.
- Implement crisis intervention techniques when dealing with emergency situations.
- Provides crisis assessments under the supervision of the on-call psychiatrist. Gather clinical information and staff with on-call psychiatrist.
- Serve as a resource to Hamilton Center staff and community regarding available services, referral sources, emergency detention procedures, etc.
- Participate in cross-training as assigned.
- Ability to demonstrate competent use of Electronic Medical Record
- Assist with answering inquiring phone calls, respond to requests for information / referral sources and route calls as appropriate.
- Review referrals from outside agencies for HCI’s inpatient unit and staff accordingly with on-call provider’s orders.
- Maintain a high level of ethical conduct regarding confidentiality, dual-relationships and professional stature.
- Keep supervisor informed of potential crisis situations.
- Maintains basic computer skills and knowledge in order to access client information and / or update information.
- Performs other duties as assigned.
Minimum Qualifications
- Knowledge and skills in the following areas : nursing care, psychiatric illnesses, dynamics of interpersonal relationships, nursing process, physical and psychological assessment skills, treatment planning, case management, basic pharmacology, administration of medication, operation of routine medical equipment, crisis intervention techniques and emergency care.
- Good written and verbal skills, record keeping, time management and interviewing skills.
- Good interpersonal skills, communication skills, and the ability to function as a team member.
Certificates, Licenses, Registrations
- Licensed by the State of Indiana as a Registered Nurse.
- Must maintain current and active license to practice, without lapse.
- Maintain current American Red Cross CPR / First Aid / AED certification.
- Maintain current Crisis Prevention Intervention (CPI) certification.
- Maintain TB screening certification.
Conditions of Employment
- Willingness to work overnights, weekends, and / or holidays in order to provide crisis management services.
- Willingness to be on call and participate in rotating weekend / holiday schedule
- Flexibility to help cover shifts or shift changes
- Completion of tuberculin screening no later than three days prior to first day of employment and annually thereafter
- Completion of MMR, Varicella, influenza and coronavirus vaccine
- Completion of a pre-employment drug screening and completion of post-employment drug or alcohol tests upon reasonable suspicion of use.
- Satisfactory completion of an Indiana Department of Child Services criminal fingerprinting background, Indiana State Criminal History, Sex and Violent Offender Registry, Child Protection Services History, and local law enforcement agency / count sheriff checks at the time of hire and every four years (or at contract renewal when applicable) for any employee that has direct contact on a regular and continuing basis with DCS consumers.
- Completion of Center-wide orientation and ALL required paperwork prior to reporting for work
- Completion of Motivational Interviewing EBP within 60 days of hire or transfer.
- Demonstrated computer literacy through successful completion of pre-employment testing may be required.
- Completion of HCI Commitment to Quality training, Trauma Informed Care, and all required annual trainings.
- Attendance at all mandatory staff development and training
- Successful completion of a six month on the job orientation period
- Participation in payroll electronic deposit
- Adherence to Compliance Program Plan
- Satisfactory reference and background investigation checks.
