We are currently looking for an experienced Rehabilitation Specialist to join our team in San Francisco, CA. The ideal candidate will play a key role in providing top-notch patient care in the comfort of their own homes, ultimately transforming their lives for the better.
As a Rehabilitation Specialist, you will assess and treat patients under physician orders, utilizing various therapeutic techniques to restore muscle strength, coordination, body alignment, and alleviate pain.
Additionally, you will be an integral part of the interdisciplinary team, providing support and information within the field of Physical Therapy.
Responsibilities
Conduct physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluations for clients following a medically approved Plan of Care.
Complete initial assessments within forty-eight (48) hours of referral, unless alternative arrangements have been made.Conduct comprehensive assessments, including OASIS data set, within forty-eight (48) hours of referral for speech therapy services, unless alternative arrangements have been made.
Develop a therapy care plan that guides other healthcare team members involved in the patient's care. Regularly review and update it in accordance with Agency policy and client needs.
Document all findings, plans, interventions, and outcomes, and submit documentation to the Agency within seven (7) days or as per Agency policy.
Communicate plans, changes, and updates to the physician, client Case Manager, and other caregivers through the care plan, progress notes, and participation in care conferences.
Notify the office on the day changes are made regarding alterations in schedule and physician orders.
Deliver appropriate teachings tailored to the client's needs and within their learning abilities and environment. Assess the client's knowledge level and identify learning needs.
Plan, teach, supervise, and counsel clients on their physical therapy needs, promoting improvement, recovery, symptom management, and awareness of community resources.
Evaluate the effectiveness of the teaching plan and establish a care plan for others to follow in educating the client.Participate actively in interdisciplinary team meetings and other required meetings.
Share areas of expertise with and among teams, initiating interdisciplinary referrals as needed.Contribute to discharge planning and document it in the client record.
Assist in the development, implementation, and evaluation of the ongoing quality improvement process. Participate in the Clinical Record Review Committee.
Utilize OASIS assessment tool and outcome reports to enhance therapy services and client outcomes.Collaborate with the interdisciplinary team to ensure consistency in data collection methods.
Promote personal safety and maintain a safe environment for clients and coworkers. Demonstrate knowledge of safety and infection control practices by adhering to agency policies and procedures.
Identify and respond to potential safety risks.Practice safety in the use of equipment.Evaluate the safety of the environment and take proactive measures to prevent accidents and ensure safety.
Supervise relevant home care team members. Provide in-home supervision of home health aides and physical therapy assistants as directed and in accordance with Agency policy.
Evaluate employee performance and conduct timely reviews as delegated.
- Perform any other duties and responsibilities deemed necessary.
- Adhere to Agency policies, procedures, and professional and community standards. Deliver services in compliance with applicable state, federal, CHAP, and JCAHO standards.
Attend meetings and required educational programs.Take responsibility for personal and professional development, identifying learning and developmental needs to be addressed with the supervisor.
Qualifications
- Graduate of a Physical Therapy program approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
- Hold a current license / registration as a Physical Therapist in the state(s) of practice.
- Minimum of two (2) years of experience in the healthcare field, with preference given to those with home care experience.
- Possess current CPR certification.
- Demonstrate strong verbal and written communication skills, as well as good interpersonal skills.
- Show the ability to work independently with minimal supervision.
- Hold a valid driver's license with automobile insurance in accordance with state and / or Agency requirements and maintain a good driving record.
