Community Resource Specialist

Community Health Partnership
Colorado Springs, Colorado, US
$52K-$60K a year
Full-time
We are sorry. The job offer you are looking for is no longer available.

Community Health Partnership (CHP) is committed to revolutionizing the way communities collaborate by fostering an environment where people come together to rethink, redesign, and rebuild to solve complex health issues.

In partnership with local Colorado Springs community partners CHP is thrilled to expand our impact through the new Prism Community Collective.

All candidates should make sure to read the following job description and information carefully before applying.

Prism Community Collective was developed with the idea that we are stronger when we come together. As a collective, we amplify voices, advocate for change, create safe, supportive spaces, and provide essential services and resources for the Colorado Springs LGBTQIA+ community.

Our mission goes beyond providing services - we're here to offer connection, healing, and growth, especially for those impacted by the attack on Club Q.

We're looking for a passionate and driven Community Resource Specialist to help survivors, their families, and the LGBTQ+ community access the resources they need.

In this role, you'll work alongside a vibrant team, navigating resources, advocating for those in need, and managing partnerships that expand the impact of our work.

This position is funded through the Anti-Terrorism Emergency Assistance Program (AEAP), with direct supervision by Community Health Partnership’s Senior Manager of Health Equity until December 2025.

Though the community and organizational partners standing up this initiative are working to ensure the Prism Community Collective’s sustainability beyond current funding, this is currently a grant-funded position for the next 30 months.

You Are A

Scavenger Hunter. Going to great lengths to find what you need is your favorite game. You enjoy searching for the best services and resources to match the situation, needs, and intended outcomes.

You leave no stone unturned in your hunt to care and support others.

Consistent Communicator. You connect with people in ways that build trust and inspire action. Regardless, if you’re facilitating a meeting, engaging in a conversation, or helping others find their voice, you communicate with professionalism and respect.

For you, it’s not just about delivering a message, it’s about how you tell the story.

Go-Getter. If it doesn’t exist, no problem! You will jump through hoops, scale mountains, and run through traffic to make things happen! Taking care of others, ensuring that the right resources exist, and are easily accessible are what motivates and rewards you.

You make things happen!

Collaborator. Teamwork makes your world go round! You lead with a voice of advocacy and support to bring people together and build professional relationships and partnerships.

You respect and value the opinion of others and strive to find common ground to collectively join forces and collaborate to solve complex problems.

Stronger together is your mantra.

What You Will Do

  • Be the go-to collaborator with survivors, LGBTQ+ individuals, clinical and programmatic providers, and partner organizations to develop and implement support for those impacted by the Club Q attack.
  • Assess needs and develop personalized care plans that empower survivors and align with Prism Community Collective’s mission.
  • Connect survivors and their families to essential resources like counseling, food, housing, and transportation.
  • Act as a liaison and trusted advocate for survivors, and the greater LGBTQ+ community, minimizing barriers and ensuring they have access to the services they need.
  • Monitor service access, checking in with providers and agency outreach, following up as needed to ensure support is ongoing.
  • Work with the Senior Manager of Health Equity and the LGBTQ+ Health Equity Manager to identify gaps in training and education for provider organizations and assist in developing a plan to build the capacity of providers to serve Club Q survivors and the greater LGBTQ+ community.
  • Develop and maintain knowledge of the availability of culturally competent resources to meet emotional, educational, financial, mental, and physical health needs, with a specific focus on LGBTQ+ affirming providers.
  • Develop and maintain strong relationships with providers, partner organizations, and community groups serving LGBTQ+ individuals.
  • Actively advocate with, and / or on behalf of, survivors within the service provision system.
  • Host and support community gatherings, activities, and outreach efforts that foster engagement, build resiliency, and promote wellbeing for clients and the greater community.

This includes sourcing and implementing programs as needed or requested by those impacted by the Club Q attack.

  • Maintain appropriate documentation and budget information, ensuring alignment with program and grant deliverables.
  • Support the Site Director, Program and Outreach Manager, and Prism Community Collective team and volunteers, as needed.

What We’re Looking For

Bachelor’s degree and / or four years’ experience in social service or related field. Equivalent experience that demonstrates the skills and abilities needed?

We’re open to that too!

  • At least two years of experience in direct service support, peer support, customer service, and / or program management.
  • Minimum two years working with the LGBTQ+ community and a demonstrated understanding of needs and unique challenges they face.
  • Independent problem-solver who can navigate the needs of a new role and program with confidence.
  • Experience working with community-based organizations, local providers, and resources.
  • Ability to establish and maintain effective working relationships across diverse cultural backgrounds.
  • High degree of emotional intelligence with the ability to balance emotions and reactions in high stress, trauma related situations and similar circumstances.
  • Skilled in trauma-informed support using a harm-reduction approach across diverse backgrounds.
  • Fast learner, multitasker, and someone who can anticipate and meet established deadlines with ease.
  • A commitment to continuously learn about inequities that Black, Brown, and Indigenous People of Color, LGBTQ+ people, people living with disabilities, and other marginalized communities face, and to ensure all of the project’s work center their experiences.

What We Offer

  • The salary range for this position is $52,000 - $60,000 / year, depending on experience.
  • Generous benefits package Medical, Dental, Vision, 401K, vacation, and sick time.
  • Flexible work environment.
  • Be part of an organization that’s transforming community wellbeing and the way we approach complex health issues in Colorado Springs and beyond.

Don't meet every single requirement? We're committed to building a diverse, inclusive, and authentic workforce. Studies have shown that women and / or people of color are less likely to apply to a job unless they meet every qualification.

If you're excited about this role, feel a passion for our mission, this feels like it could be your dream job, but your previous experience doesn't align perfectly with every qualification, we encourage you to apply!
