Community Mental Healthcare Navigator position is an exciting opportunity to identify and address obstacles relating to mental healthcare services and supports to ensure that participating individuals and families are connected to the resources that address their immediate and long-term mental health needs and interests.

Responsibilities

  • Provide assessments for clients who have a need or interest in mental health services and connect them with appropriate referral sources.
  • Work with clients to create Care Plans that clearly define their individual goals so that they can retain autonomy over their wellness objectives
  • Continuously support clients by guiding them through scheduling appointments and completing applications with various providers and services.
  • Maintain a caseload of 30 to 40 individuals for short-term periods
  • Collaborate with mental healthcare entities and community-based organizations to promote mental health wellness within communities, including workshops, insurance benefits and enrollment activities, and to contribute appropriate resources to the program’s Digital Resource Library and Map.
  • Help improve access to quality mental health care for participating individuals and families through referrals to appropriate Rising Ground and community-based programs and services.
  • Join weekly staff meetings hosted by the VP of Community Services and coordinate scheduling of in-person activities and resources offered by the Navigator program to ensure that a client’s therapeutic services are scheduled at times in which they can benefit from other relevant services.
  • Accompany clients to any appointments they may need
  • Coordinate with HealthCare Navigators regarding applications for state-sponsored benefits to help support basic health, insurance, primary care and nutritional needs.
  • Participate in training sessions and relevant staff meetings including Rising Ground’s Health Home Care Management (HHCM), Children and Family Treatment and Support Services (CFTSS) and the Access and Referral Department.
  • Maintain accurate records in accordance with state, federal and Agency guidelines and confidentiality of all client’s health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) .

Qualifications

  • Master's Degree in Social Work or related field required.
  • NYS licensed in Social Work or Mental Health Counseling required.
  • Experienced in working with marginalized populations strongly preferred.
  • Fluent in speaking and writing in English and Spanish required.
