LPN Home Health Nurse
Roslyn, NY
Responsibilities :
- Deliver direct patient care based on the established Plan of Care
- Propose modifications to the Plan of Care to the Supervising RN in response to patient care concerns that are identified
- Create accurate and prompt documentation that aligns with the Plan of Care
- Effectively communicate with team members, patients, and families to achieve optimal patient outcomes
Requirements :
- Hold a valid license as a Licensed Practical Nurse (LPN)
- COVID vaccination is mandatory
- Graduated from an accredited nursing school
- Possess at least 1 year of clinical experience, preferably in home care or acute care settings
- Have a valid Driver's License, auto insurance, and reliable means of transportation
- Demonstrate excellent observation, problem-solving, and communication skills
- Exhibit compassion and integrity
