Job Summary
Scheduling in home-visits with clients
Current roster is approximately 35 clients located in Santa Fe and surrounding areas with 3 clients located in Cuba, NM.
Clients are required to have a documented in home-visit every 60 days to asses client and ensure all needs are being met.
Participate, implement and update clients individual plan of care.
Complete intakes with all new veterans.
Recognize changes in patient needs and responses requiring intervention. Implements care to prevent or reduce risk.
Assist in yearly flu shot clinics to administer flu shots to all employees.
Responsible to implement the nursing process, the coordination and continuum of care, patient / resident assessment, patient / resident education, triage, and various other nursing interventions.
Create and build your own schedule as you go. Because you are dealing with multiple clients and schedules this position will require you to be flexible with weekends and evenings.
Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.

Position is paid at an hourly rate plus mileage reimbursement. The nursing process includes the documentation of human responses to actual or potential health problems in the home health environment.
This practice includes dependent, interdependent and independent functions. Collaborates with patient / resident and family, other inter-professional colleagues, such as physicians, to plan, implement and evaluate care.
Demonstrates competency and practices within the full scope of nursing expertise / knowledge and utilizes appropriate age and population specific standards as designated in their assigned setting.
Cares for patients / residents in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up for patients / residents in the home setting.

Qualifications
Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
Bachelor's degree in nursing preferred.
Minimum of one year of professional nursing experience preferred.
Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and / or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
Obtains and subsequently maintains required department specific competencies and certifications. Depending on location, a valid drivers license may be required.
Basic Life Support (BLS) required. Certificate for Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) preferred.
